Parents and Students,

Please inform us of any medical of health issues/conditions that your child may have. Do not assume that we are aware of any past medical conditions. If your child has asthma, diabetes and /or allergies, we need to know the severity of it and the paperwork needs to be completed in the Health Office. The information will remain confidential and will only be disclosed to the teachers you request. 

Also, if your child is not feeling well or has any type of injury, please write a note explaining the situation and give it to your physical education teacher. We will respect your request of excusing the child from participation for up to TWO DAYS and after that we will need a doctor’s note to verify the medical condition. 

Please note if you have not completed an Emergency Card, contact the Health Office immediately. In the event of an emergency this is the only way we have to contact you.
